Niagara Region Public Health wants to know how the COVID-19 pandemic has affected your life over the past eight months.
It has launched a new Community Health Survey to check in with residents and assess their needs.
The results of the online survey will help inform future response plans, programs and services.
You can take the survey online here and participants will be entered to win a Walmart gift card.
